Form aims to make life a little easier for Stratford residents

AN ONLINE form making it easy for Stratford residents to find additional support is now available.

Stratford District Council has introduced the form which automatically makes referrals to other organisations which may offer additional help to residents according to the responses they provide.

The Customer Referral Form can be completed by residents on a device or by the council’s customer service staff when a customer rings up with a query.

Depending on their circumstances, they will be asked if they want to receive some additional tailored support and, if so, asked questions relating to finances, housing, employment and wellbeing.




Referrals are generated to partner organisations where a customer indicates they would like some help in that area. For example, a customer who has caring responsibilities would be referred to Carers Trust Heart of England for practical support around caring. Or a customer impacted by addiction would be referred to Change, Grow, Live.

SDC wellbeing spokeswoman Jo Barker said: “This service enables multiple referrals to be made from one place, getting people the support they need. Covid-19 has impacted on people’s jobs, finances, housing situations and wellbeing. We have partnered with organisations who can help with all those issues and who can make a difference to our residents’ lives.”


Domestic Abuse Service manager Rachel Shuter added: “This form is another way to highlight that support is there for you if you are experiencing domestic abuse. You are not alone, Refuge is here for you. We are pleased to be a partner organisation in this new initiative and look forward to seeing how it develops.”
